If the root suffix we want to create was o=example

Enter LDAP Password:
dn: cn=o\3Dexample,cn=mapping tree,cn=config
nsslapd-state: backend
objectClass: top
objectClass: extensibleObject
objectClass: nsMappingTree
cn: "o=example"
cn: o=example
nsslapd-backend: Example

adding new entry "cn=o\3Dexample,cn=mapping tree,cn=config"

Creating a database for o=example:

dn: cn=Example,cn=ldbm database,cn=plugins,cn=config

objectClass: nsBackendInstance
cn: Example
nsslapd-suffix: o=example
nsslapd-cachesize: -1

nsslapd-require-index: off
nsslapd-directory: /var/lib/dirsrv/slapd-ldap/db/Example
